This Notice of Privacy Practices describes how protected health information (PHI) about you may be used and disclosed and how you can access this information. Please review it carefully.
Our Legal Duty
Jennifer Logan Therapy is required by law to:
- Maintain the privacy of your protected health information (PHI)
- Provide you with this Notice of Privacy Practices
- Follow the terms of this Notice currently in effect
This Notice applies to all records of your care maintained by this practice, whether created by us or received from another provider.
How We May Use and Disclose Your Health Information
The following categories describe different ways we may use and disclose your PHI. Not every use or disclosure is listed, but all permitted uses fall within these categories.
For Treatment
We may use and disclose your PHI to provide, coordinate, or manage your mental health care, including telehealth services. This may include consultation with other healthcare providers involved in your care.
For Payment
We may use and disclose your PHI to obtain payment for services provided to you. This may include billing insurance, verifying coverage, or collecting fees.
For Healthcare Operations
We may use and disclose your PHI for practice operations, such as quality assessment, training, supervision, licensing, and administrative purposes.
Uses and Disclosures Requiring Authorization
We will not use or disclose your PHI for purposes outside of treatment, payment, or healthcare operations unless you provide written authorization, except as otherwise permitted or required by law.
You may revoke an authorization at any time in writing, except to the extent action has already been taken.
Special Situations
We may disclose PHI without your authorization in certain situations, including:
- As required by law
- To avert a serious threat to health or safety
- Abuse, neglect, or domestic violence reporting
- Health oversight activities
- Judicial or administrative proceedings
- Law enforcement purposes
- Coroners, medical examiners, or funeral directors
Disclosures will be limited to the minimum necessary as required by law.
Telehealth Services
Services are provided via secure telehealth platforms. While reasonable safeguards are used, electronic communication carries some risk. Telehealth-specific risks and benefits will be discussed as part of informed consent prior to beginning services.
Your Rights Regarding Your Health Information
You have the right to:
- Get a Copy of Your Records
- You may request access to or copies of your PHI. Requests must be made in writing. We may charge a reasonable, cost-based fee.
- Request Corrections
- You may request correction of your PHI if you believe it is incorrect or incomplete.
- Request Confidential Communications
- You may request to receive communications in a specific way or at a specific location (e.g., email instead of phone).
- Request Restrictions
- You may request limits on how your PHI is used or disclosed. We are not required to agree to all requests.
- Receive a List of Disclosures
- You may request an accounting of certain disclosures of your PHI.
- Receive a Paper Copy of This Notice
- You may request a paper copy of this Notice at any time, even if you agreed to receive it electronically.
Changes to This Notice
We reserve the right to change this Notice and make the revised Notice effective for PHI we already maintain.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services.
